CONSENT

1.    I agree to myself/my child taking part in the activities of Ad-Lib Arts.

2.    I confirm to the best of my knowledge that I/my child does not suffer from any medical condition other than those I have stated during online registration.

3.    I understand that Ad-Lib Arts accepts no responsibility for loss or damage of property or injury caused by or during attendance at any of the clubs organised activities except where such loss, damage or injury can be shown to result directly from the negligence of Ad-Lib Arts.

4.    Photographs and videos of Pupils may be used to promote the activities of Ad-Lib Arts e.g. Ad-Lib Website, newsletters, office displays, newspaper articles etc.  I give permission for this.

Please note that Ad-Lib Arts cannot be held responsible for the safety or behaviour of your child once they have left our care outwith class, workshop, rehearsal or performance hours.
